Job Summary
Provide individualized, evidence-based physical therapy to older adults in geriatric populations across independent living, assisted living, dementia, and memory care settings. Conduct comprehensive evaluations, develop and implement treatment plans, and deliver hands-on interventions including therapeutic exercise, balance training, gait training, neuromuscular re-education, and manual therapy. Promote resident safety and fall prevention, educate residents and caregivers on home exercise programs, maintain timely documentation in EMR, and collaborate with interdisciplinary teams. Required Qualifications
- Graduate of a CAPTE-accredited Physical Therapy program
- Active state PT license in good standing (or eligibility for licensure)
