Job Summary
Conduct assessments and reassessments of rehabilitation needs, then plan, implement, and supervise individualized therapeutic programs including exercises, gait training, and range of motion testing. Guide patients in techniques to improve strength, balance, and coordination while teaching family members to assist safely. Coordinate care with the Rehab Director, participate in patient case conferences, and document patient reactions and condition changes. Supervise Physical Therapy Assistants via phone conferences and perform wound care per agency policy. Complete Quarterly Record Reviews and quality assurance evaluations. Perform home visits and hourly patient care daily with reliable transportation, adapting to schedule changes due to staffing or client needs. Maintain professional licensure in Texas, hold two years of experience, and adhere to infection control and safety policies.
Required Qualifications
- Graduate of a curriculum in Physical Therapy accredited/approved by the American Physical Therapy Association, or the Committee on allied health Education of the American Medical Association, or the Council on Medical Education of the American Physical Therapy Association awarding a Bachelor's degree or higher.
- Current licensed by the state of Texas to practice as a Physical Therapist.
- Two years appropriate experience as a Physical Therapist required.
- Must have reliable transportation and agency-required liability insurance.
- Must be able to adapt to client status and needs as directed.
- Must be able to work a minimum of eight (8) hours per day while driving, standing, sitting, assisting patients movement, writing, and speaking.
- Must be able to lift 50 lbs
Desired Qualifications
- One year experience in a home health setting preferred.
