Job Summary
Assess and evaluate therapeutic, rehabilitative, and functional status while developing total plans of care. Direct physical therapy treatment and instruct patients, families, and caregivers on therapeutic appliance use and prosthetic devices. Determine priority needs, report patient reactions to physicians, and initiate programs by participating in case conferences. Supervise Physical Therapy Assistants and home health aides, prepare clinical progress summaries, and participate in discharge planning. Provide consultation to home families and deliver inservice education to nursing personnel as needed.
Required Qualifications
- degree from a baccalaureate or masters program in physical therapy approved by an accredited organization
- Licensed to practice as physical therapist within the State, if applicable
- Two years of appropriate experience as a physical therapist
- Demonstrates good verbal and written communication and organization skills
- Possesses and maintains current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
