Job Summary
Conduct thorough assessments of patients' physical capabilities to create individualized treatment strategies and develop tailored therapy plans enhancing strength and mobility. Administer therapeutic exercises, manual therapy, and rehabilitation techniques while tracking progress through regular monitoring and documentation. Collaborate with a multidisciplinary team including occupational therapists, nurses, and physicians to coordinate comprehensive care pathways. Educate patients and families on home exercise programs and lifestyle strategies to foster continuous improvement. Maintain compliance with all regulatory standards and state and federal requirements.
Required Qualifications
- Must hold a current, valid license to practice as a Physical Therapist in New York State
- Possession of a Master's Degree or Doctorate in Physical Therapy from a CAPTE-accredited institution
- Demonstrated proficiency in physical therapy evaluation and treatment methodologies specific to a rehabilitation setting
- Strong interpersonal and communication skills to efficiently work with patients, families, and a multidisciplinary healthcare team
Desired Qualifications
- A minimum of 2 years of experience in a skilled nursing facility or rehabilitation center is preferred, especially with geriatric populations
- Previous involvement in developing and leading therapeutic programs that cater to diverse patient needs is highly valued
- Experience with electronic health record (EHR) systems for efficient documentation and patient management is a plus
