Job Summary
Conduct physical therapy evaluations, develop individualized treatment plans, and document patient progress for clients recovering from injury or managing long-term conditions. Provide care for adults with post-acute, rehabilitation, or complex medical needs within a skilled nursing facility setting. Maintain an active California Physical Therapist license and BLS certification while adhering to facility and regulatory requirements. Days shift from 0830-1630 on a 5x8 schedule in Portola Valley, CA, with opportunities for direct hire, temp-to-hire, or travel assignments through Jackson Therapy Partners' network.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active California Physical Therapist license
- BLS certification from AHA or ARC
Desired Qualifications
- Experience working in a skilled nursing facility, assisted living, or long-term care setting
- Experience providing care for adults with post-acute, rehabilitation, or complex medical needs
