Job Summary
Provide physical therapy services to adult and geriatric residents in skilled nursing/long-term care settings; develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence; document care accurately and maintain facility/regulatory compliance; collaborate with therapy and nursing teams to coordinate patient-centered care; educate residents and families on safe techniques to maximize independence; participate in team meetings and care planning processes; requires Wisconsin PT license and a degree in Physical Therapy (Bachelor's/Master's/Doctoral); opportunities available for days with an emphasis on a collaborative, therapist-led environment.
Required Qualifications
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA
- Current Wisconsin Physical Therapy license
- Experience with adult and geriatric populations, preferably in skilled nursing or long-term care settings
- Experience with documentation systems; Net Health is a plus
- BLS Certification may be required from AHA or ARC
