Job Summary
Physical Therapist to provide patient-focused care, helping patients move better, feel better, and stay active across home health settings. Requires a PT degree (Bachelor's, Master's, or Doctoral) and an active state license. BLS may be required. Home Health and SOC visit experience are preferred. Location is Glendale, AZ with Phoenix-area driving advantageous. Required Qualifications
- Bachelor's/Master's/Doctoral Degree in Physical Therapy from an accredited program
- Active State License to Practice Physical Therapy
- BLS certification may be required by AHA/ARC
- Home Health experience preferred (strongly)
- Experience with Start of Care (SOC) visits preferred
