Job Summary
Provide person-centered physical therapy to residents in a SNF/long-term care setting. Assess patient condition and functional limitations, evaluate tests, develop and implement individualized rehabilitation plans, administer treatments to relieve pain and restore function, monitor responses and adjust approaches with the interdisciplinary team, document thoroughly per policy and regulations, and educate patients, families, and caregivers. Collaboration with the interdisciplinary team, attendance at required meetings, and commitment to quality patient care are essential. New graduates are welcome to apply.
Required Qualifications
- Active and valid Texas Physical Therapist license
- In good standing with regulatory agencies
- SNF or long-term care experience preferred
- Net Health Electronic Documentation System experience preferred
