Job Summary
Provide 1:1 home-based physical therapy visits to patients recovering from injury, illness, surgery, or hospitalization in the Springfield, MO area. Conduct diagnostic tests and measurements of mobility, joint range of motion, strength, balance, and safety; implement wound care and protective interventions as applicable; develop and revise individualized care plans including manual therapy, gait training, balance, and other interventions; educate clients and family members in rehabilitative care to promote health and independent living; document observations and interventions on a state-of-the-art tablet; travel locally to designated cases as part of a larger interdisciplinary care team; collaborate with nursing and other professionals to manage the care plan; and participate in ongoing training and professional development.
Required Qualifications
- Active and current PT license
