Job Summary
Evaluate patients, develop individualized treatment plans, and deliver care across outpatient, inpatient, subacute, and wound-care settings within Pipeline Health hospitals. Collaborate with interdisciplinary teams, providers, and allied health professionals to coordinate discharge recommendations and manage patient caseloads. Conduct comprehensive evaluations, educate patients on their care, and delegate cases appropriately to assistants and students. Maintain accurate EMR documentation, monitor equipment inventory, and ensure compliance with HIPAA and accreditation standards. Participate in departmental meetings, attend monthly training, and implement quality improvement initiatives to enhance patient outcomes.
