Job Summary
Consult with patients to assess physical condition and functional limitations, then evaluate test results to develop individualized treatment plans and personalized rehabilitation programs. Administer medically prescribed physical therapy treatments to relieve pain and restore function while ensuring all interventions align with the established plan of care. Monitor patient response to modify approaches as clinically appropriate in collaboration with the interdisciplinary team, providing education to patients, family members, and caregivers. Complete documentation thoroughly and timely in compliance with state and federal regulations. Maintain an active Texas Physical Therapist license in good standing with regulatory agencies.
Required Qualifications
- Active and valid Texas Physical Therapist license
- In good standing with all regulatory agencies and licensing boards
- Current knowledge of physical therapy treatment practices
- Ability to manage patients with varying personalities and needs
- Strong communication, collaboration, and clinical decision-making skills
Desired Qualifications
- Skilled Nursing Facility (SNF) or long-term care experience preferred
- Experience with Net Health Electronic Documentation System preferred
