Job Summary
Provide physical therapy services to adult and geriatric residents in a skilled nursing facility in Sauk City, WI. Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence. Support residents in achieving rehabilitation goals and milestones. Accurately document care and maintain compliance with facility standards. Collaborate with therapy and nursing teams to ensure patient-centered outcomes. Educate residents and families on safe techniques and strategies to maximize independence. Participate in team meetings and contribute to care planning.
Required Qualifications
- Doctoral, Master’s, or Bachelor’s degree in Physical Therapy from an accredited program approved by the APTA
- Current Wisconsin Physical Therapist license
- BLS certification from the American Heart Association or American Red Cross
- Experience working with adult and geriatric populations, preferably in a skilled nursing facility or long-term care setting
- Clinical documentation experience. Experience with Net Health is a plus
- Authorization to work in the United States
