Job Summary
Evaluate patients within 24 hours of physician referral; develop and obtain approval for treatment plans; treat patients per physician plan; supervise PT assistants and students; train nursing staff and families for discharge; participate in care conferences and in-services; maintain documentation per state practice act and third-party payer requirements; assist with equipment orders to support mobility; communicate with supervisors and health team members about patient progress; ensure licensure and continuing education requirements are met.
Required Qualifications
- Licensed as a Physical Therapist in the state
- Bachelor’s degree in Physical Therapy; Masters or Doctorate preferred
- Must maintain license in good standing with the state
- Proficient in Casamba and Point Click Care
- Ability to read technical procedures and policy manuals
- Ability to supervise PT assistants, aides and students
- Communicate effectively with patients, families, and health team members
- Strong documentation and record-keeping skills
- Ability to participate in discharge planning and family training
