Job Summary
Rehabilitation Physical Therapist to evaluate, treat, and support patients working to restore mobility, reduce pain, and improve functional independence. Responsibilities include comprehensive patient evaluations, developing and implementing individualized treatment plans focused on mobility restoration, strength, balance, and pain management, providing manual therapy, neuromuscular re-education, gait training, and functional mobility exercises, treating orthopedic, neurological, post-surgical, and chronic conditions, monitoring progress, collaborating with Occupational Therapy, Speech Therapy, Nursing, and Care Management, educating patients and caregivers on home exercise programs and prevention, and maintaining EMR documentation in compliance with regulatory standards.
Required Qualifications
- Active South Carolina Physical Therapist License
- Minimum 2 years of rehabilitation experience (inpatient rehab, outpatient rehab, or post-acute preferred)
- Proficiency with EMR documentation
- Excellent communication, clinical reasoning, and patient education skills
- Strong knowledge of functional mobility, gait mechanics, and neuromuscular retraining
- Experience treating orthopedic, post-surgical, geriatric, and neurological patient populations
