Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, including evaluations, individualized plans of care, and evidence-based treatment. Deliver therapy to improve mobility, independence, and safety, with telehealth/virtual care as appropriate. Document in FOX’s EMR, collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), and educate patients, caregivers, and staff to promote aging-in-place. Flexible employment types include in-home & senior living care (full-time, part-time, PRN), with H1B sponsorship available for qualified clinicians. Drive to patient homes and adapt care to home-based settings.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
