Job Summary
Provide one-on-one physical therapy to older adults wherever they call home (private residences or senior living communities), including evaluations, development of individualized plans of care, and evidence-based treatment to improve mobility and independence. Deliver therapy sessions that support wellness and functional independence, document effectively in FOX’s EMR per regulatory standards, and collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists). Educate patients, caregivers, and senior living staff to promote safety and aging in place, with optional telehealth/virtual care to extend access. Employment options include Full-time In-Home & Senior Living Care (salaried with benefits), Part-time (salaried with benefits eligibility based on hours), and PRN/FOX Flex (pay per unit); H1B sponsorship available for qualified clinicians; new graduates eligible for FOX Emerging Professionals Mentor Program.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
