Job Summary
Provide evidence-based geriatric rehabilitation to improve patient mobility and function. Develop and implement individualized treatment plans, document progress, and educate patients and caregivers. Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible for licensure
- Experience with geriatric rehabilitation preferred
- Ability to develop and implement individualized treatment plans
- Strong documentation and communication skills
- Ability to educate patients and caregivers
- Ability to collaborate within an interdisciplinary team
