Evaluate, plan, and deliver individualized evidence-based functional rehab to older adults in homes and senior living communities. Improve strength, balance, and mobility; prevent falls; and promote independence through autonomy with strong clinical support, mentorship, and productivity tools. Collaborate with an interdisciplinary team, document in EMR, and uphold high ethical and clinical standards within a mission-driven, clinician-founded practice.
