Job Summary
Evaluate patients, establish diagnoses and prognoses, and implement treatment programs in coordination with medical staff and support personnel. Direct and supervise Physical Therapy services, including support staff, while consulting with referring providers to clarify instructions. Document patient information into charts per APTA and insurance guidelines, maintain records, and review and co-sign Physical Therapist Assistant notes. Instruct patients, families, and caregivers on activities to optimize services, select appropriate billing codes, and maintain quality assurance and infection control protocols. Maintain the unit's cleanliness and equipment condition, conduct educational programs, and assist with data collection and reporting. Work Monday through Friday from 7:00am to 6:00pm with no nights or weekends required.
Required Qualifications
- Minimum of Bachelor's Degree in Physical Therapy
- Graduation from accredited Physical Therapy Program
- License to practice physical therapy in state of Illinois
- CPR Certification
- Ability to sit, stand, walk, bend, stoop, stretch and lift up to 75 pounds
- Ability to use department equipment
- Ability to facilitate prescribed treatment
- Ability to handle patients
- Ability to communicate nature of treatment to patient
- Ability to prepare provider's report
- Hearing within, or correctable to, normal range
- Vision correctable to 20/20
- Manual dexterity for the operation of office equipment
- Monday-Friday availability
- Hours between 7:00am-6:00pm
- No nights or weekends
