Job Summary
Evaluate assigned patients by gathering data from records, interviews, and observations. Develop and implement individualized treatment programs to promote health and achieve goals, modifying strategies based on ongoing evaluations. Provide direct therapy services across various functional settings, coordinating care with rehabilitation, medical, and nursing teams. Prepare required documentation including evaluation results, treatment plans, and progress notes. Communicate effectively with patients, families, and interdisciplinary members regarding status and needs. Establish rapport and cooperate throughout the intervention program while enhancing professional growth through educational participation.
Required Qualifications
- Active license in the state of practice upon hire
- Visual acuity adequate to perform position duties
- Ability to communicate effectively with others
- Ability to hear, understand and distinguish speech and other sounds
- Ability to work the hours specified
Desired Qualifications
- 1-3 years Experience as a licensed physical therapist
