Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort and companionship. Deliver a holistic approach supporting physical, spiritual, emotional, and clinical needs across specialty programs including urinary incontinence, vestibular rehab, home safety assessment, movement disorders, and low vision. Complete documentation to meet reimbursement and regulatory requirements within a resident-centered environment. This per diem role offers onsite physician support, interdisciplinary team collaboration, and access to wellness visits for staff.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
- Experience with seniors in home care setting
- Experience with OASIS
