Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort, companionship, and belonging. Deliver holistic support addressing physical, spiritual, emotional, and clinical needs across specialty programs including urinary incontinence, vestibular rehab, and home safety assessment. Complete documentation to meet reimbursement and regulatory requirements. This per diem role offers onsite physician access, interdisciplinary team collaboration, and competitive compensation at $55 per hour within an award-winning campus in Tinton Falls.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
- Experience with seniors in home care setting
- Experience with OASIS
