Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort and companionship. Deliver a holistic approach supporting physical, spiritual, emotional, and clinical needs across specialty programs including urinary incontinence, vestibular rehab, and home safety assessments. Complete documentation to meet reimbursement and regulatory requirements with a valid state Physical Therapy license.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
- Experience with seniors in home care setting
- Experience with OASIS
