Job Summary
Conduct evaluations and deliver personalized treatment across a full continuum of care for older adults within a single on-site campus. Provide 1:1 direct patient care focused on helping residents maintain mobility, independence, and quality of life while collaborating with nursing, physicians, and rehab colleagues. Accurately document services on time and work within an interdisciplinary clinical team to address resident needs throughout their care journey. No outside travel is required as all clients reside on the 63-acre campus.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- Ability to document services accurately and on time
- Must be able to lift 50 lbs
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
