Job Summary
Conduct evaluations and deliver personalized treatment across a full continuum of care for older adults on a 1:1 direct patient care focus. Document services accurately and on time while working alongside nursing, physicians, and rehab colleagues to help residents maintain mobility, independence, and quality of life. Expand clinical expertise within an interdisciplinary clinical team at a 63-acre campus where all clients reside on-site, eliminating the need for outside travel.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- Ability to document services accurately and on time
- Must be available for predictable weekday schedule
- Must be able to work within a 1:1 direct patient care model
- Must be able to work on-site with no additional travel
- Must be able to work in a caseload focused environment
- Must be able to work alongside nursing, physicians, and rehab colleagues
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
