Job Summary
Implement residents' personal care plans while supporting their physical, spiritual, emotional, and clinical needs. Complete documentation to meet reimbursement and regulatory requirements. Provide therapeutic services to older adults across multiple diagnostic groups, focusing on residents' strengths, preferences, and preferred routines. Offer companionship and belonging within a resident-centered environment. This per diem role offers a competitive rate of $55 per hour with onsite medical centers and continuing education assistance.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
- Experience with seniors in home care setting
- Experience with OASIS
