Physical Therapist

Conduct comprehensive physical therapy evaluations and treatments for residents in a skilled nursing setting, administering interventions to improve mobility, strength, balance, and overall function. Develop and implement individualized treatment plans based on resident needs and physician recommendations while maintaining accurate clinical documentation, progress notes, and discharge summaries. Collaborate with interdisciplinary team members to coordinate care and adjust treatment plans as resident progress warrants. Maintain statistical records of treatments and submit requisitions for supplies to the Director Physical Therapy. Assume all duties of the Director Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for necessary documentation. Requires a current State Physical Therapy License, graduation from an approved school, and two years' experience preferred.
