Physical Therapist

Physical Therapist position in an on-site Edgerton, WI setting. Responsibilities include performing initial patient examinations, evaluating referrals, creating and revising the plan of care, maintaining patient records, and ensuring delivery of quality therapeutic services within the rehab department. Requires graduate of an accredited physical therapy program and state licensure; CPR preferred. The role supports an in-house therapy program with professional standards per AOTA and state practice acts, and involves collaboration with clinical leadership; EMR documentation and payer-source knowledge are important.
