Evaluate and treat residents in an assisted living community or home setting, developing and implementing individualized treatment plans for hospice patients. Collaborate with physicians, families, and the interdisciplinary care team while maintaining accurate, timely documentation in the EMR. This role offers a flexible PRN schedule with clinical autonomy, allowing you to choose assignments that fit your availability without driving between homes. Required Qualifications
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
- Active Florida Physical Therapist (PT) license
