Physical Therapist ( DPT) @ Durham Fidelity

The Physical Therapist provides evidence-based care to promote an optimal patient experience. The Physical Therapist will be allotted 45 minutes one-on-one with all new and returning patients and treats a variety of diagnoses including repetitive stress and athletic injuries. Works within an integrated care team to optimize patient outcomes and clinic performance metrics, documents accurate patient findings in the shared EMR, and develops new programs to enhance the patient-care experience while adhering to ethical and legal standards defined by the profession. Responsible for collaborating with the physical medicine rehab team and comprehensive care team, implementing clinical standards of care and workflows, and contributing to program development and performance improvement activities. The role emphasizes patient-centered care within an interdisciplinary environment and may require standing, walking, and assisting with patient care as needed.
