Physical Therapist

Part-time Physical Therapist needed to plan, develop, organize, implement, evaluate, and direct physical therapy services and programs in a skilled nursing facility, with travel between the Bloomer campus and the Regional Vent Center in Chippewa Falls. Approximately 20 hours per week with a flexible schedule. Responsibilities include discharge planning, patient care plan development, patient assessments, maintaining treatment records and progress notes, collaborating with and supervising PTAs, and ensuring compliance with current rules and guidelines. Requires a Doctorate in Physical Therapy (or eligible grandfathered bachelor’s/master’s degree in physical therapy) and current Wisconsin PT license.
