Develop personalized plans of care for diverse patients with orthopedic and neurologic diagnoses while objectively measuring outcomes using cutting-edge software. Deliver telehealth visits, assessments, and treatment plans via digital platforms, and perform Remote Therapeutic Monitoring services including interactive communication. Efficiently document evaluations, treatments, re-evaluations, and discharge notes. Serve as a liaison between patients, in-clinic providers, and the Director of Digital Health to optimize remote services and support operations. Maintain regular reporting and data analysis of regional service metrics while participating in program development and marketing efforts. Work full-time hybrid hours (40 hours/week, 5 days/week) with three days in the Tampa clinic and two days remote. Fluent in English and Spanish required.
