Job Summary
Provide evidence-based physical therapy to SNF/long-term care residents in Plainview, TX. Evaluate residents and develop individualized treatment plans based on functional needs and physician orders; administer PT treatments to relieve pain, improve mobility, restore function, and promote overall wellness. Establish measurable goals, modify treatment plans based on patient progress, and collaborate with physicians, nursing staff, therapy team members, residents, and families to ensure optimal outcomes. Educate residents, family members, and caregivers; supervise Physical Therapist Assistants and rehabilitation support staff as appropriate; ensure services align with the plan of care and regulatory requirements; complete accurate documentation; participate in interdisciplinary care planning; opportunities for professional growth and new graduates are welcome.
Required Qualifications
- Active and unrestricted Texas Physical Therapist (PT) license
- Graduate of an accredited Physical Therapy program
