Physical Therapist

Provide skilled physical therapy treatments to patients with injuries, illnesses, and functional limitations to help them achieve independence in the comfort of their homes. Conduct assessments, record findings, and develop individualized treatment programs with measurable goals that align with physician plans and patient objectives. Design and implement rehabilitation programs, adapting as needed for optimal results, while supervising Physical Therapy Assistants to ensure consistent, high-quality care delivery. Maintain timely, accurate documentation of patient care and progress, communicating effectively with patients, families, and team members to report progress and update care plans.
