Job Summary
Conduct ongoing interdisciplinary assessments and develop individualized plans of care in partnership with patients, caregivers, and physicians. Deliver physical therapy services including evaluating therapeutic status, assessing home environments, and directing treatment to meet physician-approved goals. Provide patient, caregiver, and family counseling while educating them on therapeutic appliance use and prosthetic devices. Prepare clinical notes and progress summaries, communicate treatment reactions, and participate in discharge planning and peer consultation processes. Supervise Physical Therapy Assistants and home health aides as required by policy and regulations.
Required Qualifications
- Licensed to practice as physical therapist within the State, if applicable.
- Passed an examination for physical therapists approved by the state
- Possesses a degree from a baccalaureate or master's program in physical therapy approved by the Commission on accreditation in Physical Therapy Education (CAPTE) or any successor organization of the CAPTE
- Two years of appropriate experience as a physical therapist
- Demonstrates good verbal and written communication and organization skills
- Possesses and maintains current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
- Based in Oklahoma City and operating in various counties throughout the state
Desired Qualifications
- Community/home health experience is preferred
- HCHB/POINTCARE EXPERIENCE IS A PLUS
