Physical Therapist

Evaluate patients with various diagnoses, establish treatment plans, and formulate goals. Perform procedures, modalities, ADL activities, and exercise programs while fabricating orthotics as needed. Maintain accurate patient records including progress notes, evaluations, discharge summaries, and billing forms. Supervise Physical Therapy assistants through verbal or written instruction, review their documentation, and provide feedback on treatment progress. Instruct patients and families on home exercises and provide necessary written materials. Participate in department quality improvement and safety programs, attend management meetings, and follow all policies regarding safety, documentation, and infection control. Assist with equipment maintenance and accept additional assignments within scope.
