Physical Therapist

Evaluate patients with various diagnoses, establish treatment plans, and formulate goals. Perform procedures, modalities, ADL activities, and exercise programs indicated by patient condition, including fabricating orthotics. Maintain accurate patient records, progress notes, and billing forms. Supervise Physical Therapy assistants through verbal or written instruction, review their documentation, and provide feedback on treatment progress. Instruct patients and families on home exercises and provide written materials. Participate in department quality improvement and safety programs, attend management meetings, and follow all safety and infection control policies. Assist in equipment maintenance and accept other assignments as needed.
