Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, balance, and overall physical function, adjusting treatment plans as needed to achieve optimal outcomes. Collaborate with the interdisciplinary home health team, including nurses and occupational therapists, to provide coordinated care. Educate patients and caregivers on exercises, safety, and assistive device use while documenting all visits and progress notes accurately. Maintain a Master's degree in Physical Therapy and current state licensure, with CPR certification and a valid driver's license required. Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
- This is a safety-sensitive position

Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
