Job Summary
Deliver skilled physical therapy to patients in home health based on an individualized Plan of Treatment developed with the referring physician; evaluate patient needs and establish rehabilitation goals to improve mobility, strength, balance, and overall function; adjust treatment plans as needed; collaborate with an interdisciplinary home health team (nurses, OTs, SLPs, social workers) to provide coordinated care; educate patients and caregivers on exercises, safety, mobility strategies, and assistive devices; document visits, assessments, and progress notes; support patients to maximize independence and quality of life wherever they call home.
Required Qualifications
- Master’s degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification required
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver’s license, auto insurance, and reliable transportation
