Job Summary
Responsible for evaluating patients and developing/implementing individualized physical therapy treatment plans across inpatient, outpatient, and home health settings. Performs data collection from patient records, interviews, observations, and team members; conducts patient and family instruction for home programs or exercises; designs interventions to promote health and meet treatment goals; communicates status with patients, families, physicians, and the care team; supervises and coordinates with physical therapy assistants, technicians, and students; maintains documentation (evaluation results, treatment plans, progress notes); participates in care conferences and professional development; adheres to equipment usage knowledge and safety standards; collaborates with interdisciplinary team to ensure safe, effective patient care and efficient department operations. Essential qualifications include the ability to work the specified hours and an active license in the state of practice; preferred qualification includes licensed physical therapist.
