Job Summary
Evaluates and conducts medically prescribed physical therapy treatment programs to treat movement dysfunction and pain resulting from injury, disease, disability, or other health-related conditions. PRIMARY RESPONSIBILITIES include performing initial and ongoing assessments of patient condition; establishing, revising and evaluating a plan of care; delivering therapy interventions using standard physical therapy techniques; educating patients/families and staff; coordinating discharge planning, home instructions, equipment/home modifications, and follow-up care; supervising physical therapy assistants, students, and support personnel; maintaining documentation on patient response and intervention; and adhering to safety standards while performing other duties as assigned. EDUCATION qualifications require a Bachelor’s, Master’s or Doctorate degree in Physical Therapy (or related as required for licensure). LICENSES include BLS HCP within 90 days and state licensure in IL, MO, OK, and WI. EXPERIENCE: No experience required. PHYSICAL and WORK REQUIREMENTS describe standing, lifting, mobility, driving, and other activities typical for an on-site clinical PT role. Job Type: Employee; Schedule/Shift: Day shift; 40 hours/week; Location: Mount Vernon, IL with licensure in IL/MO/OK/WI.

- Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) within 90 days of employment
