Job Summary
Physical Therapist for BoldAge PACE provides comprehensive therapy services to seniors in home, clinic, and center settings. Plan, organize, and implement PT services including initial assessments, treatment planning and execution, education for participants and families, and collaboration with the Interdisciplinary Team to create individualized care plans. Responsibilities include home safety assessments, equipment need evaluations, documentation of evaluations and plans, training PACE staff on transfers and ADLs, participation in quality improvement initiatives, and maintaining compliance with policies and OSHA guidelines. Requires an advanced degree in Physical Therapy, state licensure, and driving insurance; prior experience on an IDT and with frail elderly populations is preferred.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver’s license and proof of auto insurance
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
