Job Summary
Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include assessment, planning, providing treatment, and teaching participants, caregivers or family to maintain participant support in the community. Serves on the IDT, performs in-person assessments, evaluates need for home safety adaptations and equipment, develops treatment plans, documents care, educates participants and families, and collaborates with the care team to support optimal independence and quality of life for seniors.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver’s license and proof of auto insurance
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
