Job Summary

Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include assessment, planning, providing treatment, and teaching participant, caregiver or family to maintain participant support in the community. Provides high quality care as a member of the IDT; conducts in-person assessments at enrollment and as needed; evaluates need for adaptive equipment and home safety; plans and administers PT treatment programs per standards; documents evaluations and treatments; educates participants and families; trains PACE staff in body mechanics and safe transfers; delivers care in center, contracted facilities, and homes; collaborates on comprehensive care plans; participates in IDT meetings and quality improvement; maintains licensure, continuing education, and related professional requirements. Ensures privacy and safety, follows OSHA guidelines, and engages in ongoing professional development.

Required Qualifications

  • Advanced degree in Physical Therapy from an accredited school of Physical Therapy
  • Current license to practice Physical Therapy in the state
  • Current driver’s license and proof of auto insurance
  • Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
  • 1 year of experience working with a frail or elderly population preferred
