Job Summary
Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, memory care, etc.) within the FOX Rehabilitation Geriatric House Calls model. Develop evaluations and individualized plans of care, deliver evidence-based treatment, document in the EMR, collaborate with an interdisciplinary team (PT/OT/SLP/fitness), and educate patients, caregivers, and staff. Telehealth/virtual care is optional to extend access and flexibility. Roles available include Full-time, Part-time, and PRN/Home-visit pathways in the Commack, NY area with H1B sponsorship available for qualified clinicians; new graduates eligible. The position emphasizes autonomy, flexible scheduling, and meaningful patient outcomes in in-home and senior living settings, with a focus on aging in place and mobility/ functional independence.
Required Qualifications
- Valid Physical Therapy license in the state of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
