Job Summary
Design and deliver personalized 1:1 rehabilitative therapy programs for patients recovering from surgery, illness, or accident, assessing progress by being part of their day-to-day life. Conduct comprehensive assessments to determine patient needs and develop treatment plans and outcomes. Deliver therapy to patients in their home or care facilities while constantly evaluating evolving needs to adjust plans of care. Practice independently in the community as part of an interdisciplinary care team.
Required Qualifications
- Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern
- Licensure to practice as a Physical Therapist in New York State
- Valid driver's license required for all areas outside the borough of Manhattan
