Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs, adjust treatment plans, and educate patients and caregivers on exercises and safety strategies. Document all visits, assessments, and progress notes accurately. Collaborate with the interdisciplinary home health team including nurses and occupational therapists. Requires Michigan PT licensure, Master's degree, and CPR certification. Full-time role with $7,500 sign-on bonus, fleet car program, and comprehensive benefits.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
- PT Licensure - Michigan
- Must be able to lift 50 lbs
Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
