Job Summary
Conduct daily patient evaluations, select and administer tests, and synthesize results to formulate diagnoses, goals, and treatment plans. Execute musculoskeletal interventions including therapeutic exercise, joint mobilization, and modalities while providing education on ADLs, transfers, and community reintegration. Record all care data and billing in the Prompt EMR system, oversee support staff, and collaborate with the interdisciplinary team to modify plans of care. Manage discharge planning and referrals as needed.
Required Qualifications
- Bachelor's or Master's Degree
- Doctorate degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
