Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed with referring physicians. Evaluate patient needs to establish rehabilitation goals and adjust techniques to improve mobility, strength, and balance. Collaborate with interdisciplinary home health teams including nurses and occupational therapists to provide coordinated care. Educate patients and caregivers on exercises, safety strategies, and assistive device usage. Document all visits, assessments, and progress notes accurately. Work Monday through Friday from 8am to 5pm in Ft. Wayne, IN. Requires a Master's degree in Physical Therapy and current IN licensure.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
- Monday-Friday, 8am-5pm
- Physical Therapy - Licensure in IN
Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
