Job Summary
Plan, organize, and implement physical therapy services for PACE participants, including in-person assessments at enrollment and ongoing evaluations. Assess needs for adaptive equipment, recommend home modifications, and administer treatment programs consistent with recognized standards. Confer with the interdisciplinary team to develop comprehensive care plans, document evaluations and treatments accurately, and measure treatment effectiveness. Educate participants and families on procedures and home programs, while training staff on body mechanics, transfers, and gait patterns. Provide care in centers, clinics, contracted facilities, and participants' homes, actively participating in daily team meetings and quality improvement projects. Maintain a safe working environment, follow OSHA guidelines, and complete all required continuing education and certifications.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver's license
- Proof of auto insurance
Desired Qualifications
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting
- 1 year of experience working with a frail or elderly population
- Training will be provided upon hiring (If applicable for the role)
